At the start of the year we often confer with the student on the mini-lesson focus or perhaps an obvious need (ex. stay in one spot, picking just right books, etc.); as the year goes on we want to target instruction to the individual which means helping students set reading goals.  Above are various goal lists that you can pull from to create your own "cheat sheet" for conferring.
